Job Overview

Position Summary
The Family Support Navigator role focuses on delivering comprehensive support services to families with children, aiming to bolster protective factors and enhance adult capabilities to foster child welfare and development. This position is dedicated to providing individuals, families, and relative caregivers with pertinent information and referrals tailored to their specific needs.

Key Responsibilities
  • Conduct home visits to families to offer personalized guidance, develop support strategies, provide resources and referral services, and demonstrate educational activities.
  • Offer coaching to families to help them achieve their self-defined objectives.
  • Maintain accurate records for assigned families and supply necessary data for reporting.
  • Engage in community outreach to connect with kinship caregivers and assess their needs.
  • Assist in formulating tailored Family Support Plans.
  • Help individuals, families, and relative caregivers access various benefits and services, including DHHS benefits, educational system navigation, transportation assistance, and more.
  • Facilitate group sessions and actively participate in community events.
  • Stay informed about current resources that may benefit participants.
Qualifications
  • High School diploma or equivalent.
  • Experience working with individuals and families within the community.
  • Valid driver's license, along with a criminal background check and DMV record check.
Preferred Skills
  • Ability to provide strength-based family support.
  • Familiarity with community services available for families.
  • Experience working with diverse populations.
  • Strong communication and time management skills.
  • Capability to maintain organized files and uphold confidentiality.
