Position Overview

The Registered Nurse (RN) at Zufall Health is a licensed professional dedicated to delivering exceptional patient care. Reporting to the Senior Vice President of Clinical and Nursing Operations, the RN utilizes specialized expertise, clinical judgment, and nursing skills to provide comprehensive care to patients while adhering to the highest standards of professional nursing practice in a clinical environment. The RN collaborates closely with patients, families, healthcare providers, and office personnel to ensure the delivery of patient-centered, cost-effective, and high-quality healthcare services. This role is pivotal in community health, focusing on care coordination and management, supervising clinical support staff, and actively participating as a key member of the Care Team. This is a full-time position encompassing all nursing responsibilities associated with primary care services, including those delivered in clinic settings, mobile health units, and community outreach initiatives.

Key Responsibilities

  • Collaborate with Care Teams, healthcare providers, outreach personnel, and clinical support staff to optimize patient care services and outcomes.
  • Perform triage duties in person or via telephone during clinic hours, addressing medical inquiries and managing patient flow according to established protocols.
  • Implement and uphold clinical policies and procedures in accordance with Zufall Health Center guidelines.
  • Oversee laboratory services, ensuring compliance with CLIA/CLIS regulations, including point-of-care testing.
  • Act as a liaison for immunization programs, ensuring accurate data entry and reconciliation of vaccines.
  • Review laboratory and diagnostic results, promptly addressing those requiring immediate attention.
  • Provide education and counseling on primary care services, including vaccination protocols and women's health issues.
  • Assess community health needs and coordinate responses to public health concerns.
  • Participate in health fairs and community outreach events.
  • Educate patients on procedures and tests, ensuring informed consent is obtained when necessary.
  • Document patient interactions and care plans in the electronic medical record system.
  • Assist patients with administrative tasks related to healthcare access and benefits.
  • Advocate for patients within community organizations and governmental agencies.
  • Prepare reports for grant analysis and compliance with center requirements.
  • Maintain patient confidentiality and adhere to all regulatory standards.
  • Perform additional duties as assigned, including assuming the responsibilities of the Director of Nursing when necessary.

Qualifications

  • Completion of an accredited nursing program and successful passage of the National Council Licensure Examination for Registered Nurses.
  • Current and valid RN license in New Jersey.
  • Current Basic Life Support (BLS) certification.
  • Minimum of one year of experience in an ambulatory care or community health setting.
  • Proficiency in Microsoft Office and electronic medical records.
