About the Role

Family Health Centers of San Diego is seeking a skilled and compassionate LVN Home Care PACE Oceanside to join our team. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality care to our patients in their homes.

Key Responsibilities
  • Conduct thorough home visits to assess patient needs and develop personalized care plans.
  • Provide accurate and timely documentation of patient care, ensuring seamless communication with the interdisciplinary team.
  • Collaborate with patients, families, and caregivers to educate and empower them to take an active role in their care.
  • Participate in quality management initiatives to ensure the highest standards of care are met.
  • Meet productivity standards while maintaining a high level of accuracy and attention to detail.
  • Coordinate home care delivery, ensuring the timely and effective implementation of care plans.
  • Communicate effectively with patients, families, and caregivers, addressing their concerns and needs in a compassionate and professional manner.
  • Act as a liaison between the interdisciplinary team, caregivers, and community agencies, fostering strong relationships and ensuring seamless care coordination.
  • Provide ongoing education and support to patients, families, and caregivers, promoting health literacy and self-advocacy.
  • Participate in staff meetings, contribute to care planning, and communicate changes in patient condition to the team.
  • Participate in unskilled staff competencies, evaluations, education, and guidance, providing valuable insights and expertise.
  • Assist in the development and implementation of policies and procedures, ensuring alignment with organizational goals and standards.
Requirements
  • Current California LVN License.
  • Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training.
  • Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required.
Preferred Qualifications
  • 1-2 years of experience working with the older adult population.
  • Clinical competency with the older adult population.
  • Ability to work effectively in a team environment.
  • Demonstrated ability to communicate effectively and openly with other interdisciplinary team members on patient status and needs.
  • Must be able to multi-task, be flexible, ensure accuracy, and meet changing priorities in a fast-paced, high workload environment.
  • Basic computer literacy to comply with department needs and expectations.
  • Bilingual English/Spanish preferred.
Working Environment

This role requires the ability to work in a fast-paced environment, with frequent travel to patient homes within San Diego County. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
