About the Role

Family Health Centers of San Diego is seeking a skilled LVN Home Care PACE Oceanside professional to join our team. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality care to our patients.

Key Responsibilities
  • Conduct home visits to assess patient needs and develop personalized care plans.
  • Provide accurate and timely documentation of patient care and progress.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination.
  • Communicate effectively with patients, families, and caregivers to ensure understanding and compliance with care plans.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
  • Assist with scheduling and coordination of home care services as needed.
  • Ensure compliance with regulatory requirements and departmental policies and procedures.
  • Act as a liaison between the interdisciplinary team, caregivers, and community agencies.
  • Provide education and support to patients and families on health-related topics and equipment use.
  • Participate in staff meetings and contribute to the development of policies and procedures.
Requirements
  • Current California LVN License.
  • Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training.
  • Ability to travel within San Diego County, with proof of liability and property damage insurance on vehicle used.
Preferred Qualifications
  • 1-2 years of experience working with the older adult population.
  • Clinical competency with the older adult population.
  • Ability to work effectively in a team environment.
  • Effective communication and interpersonal skills.
  • Basic computer literacy to comply with department needs and expectations.
  • Bilingual English/Spanish preferred.
Working Environment

This role requires regular driving within San Diego County, with occasional bending, lifting, and standing. The ability to work in a fast-paced environment with changing priorities is essential.

Compensation

Family Health Centers of San Diego offers a competitive salary range of $36.65, exclusive of fringe benefits or potential bonuses. Final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience.
