Job Title: Home Care Coordinator

We are seeking a skilled and compassionate Home Care Coordinator to join our team at Gary and Mary West PACE. As a Home Care Coordinator, you will play a vital role in ensuring the delivery of high-quality, person-centered care to our participants.

Key Responsibilities:
  • Coordinate home care schedules and allocate resources to meet the needs of our participants.
  • Work closely with our Personal Care Supervisor to ensure seamless care delivery and effective communication.
  • Assess and prioritize the needs of our participants, ensuring timely and accurate scheduling of home care services.
  • Collaborate with our interdisciplinary team to develop and implement care plans that promote the health, well-being, and independence of our participants.
  • Provide exceptional customer service and maintain open communication with participants, families, and caregivers.
Requirements:
  • High school diploma or equivalent required; LVN/LPN preferred.
  • Minimum of two years of experience in home care or a related field.
  • Bilingual English/Spanish required.
  • Strong organizational and time management skills, with ability to prioritize tasks and manage multiple responsibilities.
  • Excellent communication and interpersonal skills, with ability to work effectively with diverse populations.
