Job Summary:

The HARP Supervisor will be responsible for overseeing the day-to-day operations of the HARP Integrated Care Department. This includes supervising HARP Care Manager operations, monitoring the departmental phone queue, and ensuring quality of calls between care managers, members, and providers.

Key Responsibilities:
  • Provides guidance and support to HARP Care Managers, particularly in regards to new processes and workflows that support program operations.
  • Promotes a multidisciplinary approach to care coordination, supporting HARP coordinated care operations amongst disciplines.
  • Oversees clinical program training and retraining, and creates and updates departmental training tools, workflows, and resources.
  • Ensures adherence to clinical standards of care through collaboration with providers to ensure appropriate outcomes.
  • Supervises crisis calls and escalates when necessary.
  • Maintains a tracking tool to log unacceptable inconsistencies and errors observed during quality reviews of recorded calls and care management documentation.
  • Performs quarterly audits of staff chart documentation.
  • Works collaboratively with the HARP management team to ensure program goals, projects, and initiatives are implemented and meet departmental workflows and policy standards.
  • Supports Care Management staff as needed when management is in meetings or working on other program initiatives.
Qualifications:
  • Master's Degree in Social Work (MSW), Master's Degree in a related field (MA/MS), or equivalent required.
  • Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), or Registered Nurse (RN) preferred.
  • Bilingual in Spanish preferred.
  • Two years of experience working with Behavioral Health and/or Substance Abuse required.
  • Prior supervisory experience required.
  • Previous Managed Care experience required in Medical Management/HARP Operations.
  • Knowledge of the Collaborative Care Model.
  • Experience with chronic condition management.
  • Excellent written and oral communication skills required.
  • Ability to multi-task well while maintaining a positive attitude.
  • Demonstrated ability to manage large caseloads in a fast-paced environment while building and enhancing team productivity.
  • Demonstrated professionalism and leadership skills, along with the ability to develop, direct, and support staff.
