HARP Supervisor Integrated Care

The HARP Supervisor will oversee the HARP Care Manager operations within the HARP/Health Home Division. This role will ensure the quality of calls between care managers, members, and providers. The HARP Supervisor will conduct new hire training and continued training for all clinical staff. This position will also participate in daily operations, enhancing communication and processes within the clinical and non-clinical areas.
Responsibilities- Provide guidance on new processes and workflows that support program operations within the HARP Integrated Care Department.
- Promote a multidisciplinary approach to support HARP coordinated care operations amongst disciplines.
- Oversee clinical program training, retraining, and the creation and updating of departmental training tools, workflows, and resources.
- Adhere to clinical standards of care through collaboration with providers to ensure appropriate outcomes.
- Practice and adhere to departmental and state guidelines to protect self, members, and the organization.
- Supervise crisis calls and escalate when necessary.
- Maintain a tracking tool that logs unacceptable inconsistencies and errors observed during quality reviews of recorded calls and care management documentation.
- Perform quarterly audits of staff chart documentation.
- Work collaboratively with the HARP management team to ensure program goals, projects, and initiatives are implemented and meet departmental workflows and policy standards.
- Support Care Management staff as needed when management is in meetings and/or working on other program initiatives.
- Follow best practice and clinical standards, and adhere to departmental and state guidelines.
- Perform all other duties or actions as required.
- MSW/MA/MS Masters Degree or equivalent required.
- LMSW, LCSW, LMHC, LMFT, RN preferred.
- Bilingual in Spanish preferred.
- Two years experience working with Behavioral Health and/or Substance abuse required.
- Prior supervisory experience required.
- Previous Managed Care experience required in Medical Management/HARP Operations.
- Knowledge of the Collaborative Care Model.
- Experience with chronic condition management.
- Excellent written and oral communication skills required.
- Ability to multi-task well while maintaining a positive can-do attitude.
- Demonstrated ability to manage large caseloads in a fast-paced environment while building and enhancing team productivity.
- Demonstrated professionalism and leadership skills along with the ability to develop, direct, and support staff.
Salary: $62,000-$72,000
