Element Care is seeking a skilled Registered Nurse to join our team as a Nurse Care Specialist. In this role, you will provide high-quality care to our geriatric population, working collaboratively with our interdisciplinary team to develop and implement comprehensive care plans.
Key Responsibilities- Direct Care Provision: Provide direct care to participants, including initial and periodic assessments, care plan development, and revision as needed.
- Medical Information Collection: Collect and analyze medical and social information to ensure comprehensive decisions are made regarding participant care.
- Quality Assurance: Monitor clinic care and service delivery to ensure conformance with established quality assurance and operational policies, procedures, and standards.
- Health Status Monitoring: Remain attentive to participants' health status and social functioning, identifying overt problems/needs and setting priorities.
- Care Plan Development: Coordinate with the interdisciplinary team to develop comprehensive care plans for each participant, incorporating immediate and continuing care needs.
- Test Coordination: Ensure all tests, lab work, and diagnostic studies ordered by the physician have been carried out.
- License and Certification: Registered Nurse currently licensed in the Commonwealth of MA.
- Experience: Minimum of two (2) years experience with the geriatric population.
- Technical Skills: Proficient in the use of Microsoft Office tools.
- Travel Requirements: Ability to travel locally on a frequent basis in a personal vehicle.
- Critical Thinking: Ability to quickly recognize situations/problems before they become acute, and to implement effective solutions and understand consequences.
- Covid Vaccine Requirement: Covid vaccine required.
