We are seeking a skilled Clinical Care Specialist to join our team of healthcare professionals in the Brighton residential area of Massachusetts. This Full-Time position offers a competitive annual salary of $95,000-$105,000, along with a comprehensive benefits package.

Clinical Care Specialist Responsibilities:
- Wound care
- Medication management
- Catheter management
- Diabetic Care
- Accurate and timely documentation
- Visits can range from basic wellness visits to education on disease management, follow up on MD visits and ordering supplies, wound care, medication management, etc.

Requirements:
- Candidates must have MA Registered Nurse (RN) license.
- Minimum of 1-year experience as a Registered Nurse (RN).
- Ability to work independently and as part of a team while working with a variety of patients and medical professionals
- Excellent communication and interpersonal skills
- Must possess strong organizational skills and attention to detail
- Ability to multi-task and prioritize tasks, use a computer, including electronic medical records (EMR) systems
