Job Summary:

The Dietitian Specialist serves as a vital member of the PACE Interdisciplinary Team at Element Care. This individual works under the direction of the Nutrition Coordinator and the Site Coordinator. The Dietitian Specialist provides comprehensive nutrition education and assessments to PACE participants, ensuring their nutritional needs are met.

As a key member of the IDT, the Dietitian Specialist participates in the planning, implementation, and evaluation of care plans that align with the PACE model of care. This involves collaborating with the team to achieve the PACE Mission of supporting frail elders in living safely in their communities for as long as possible.

Essential Functions:
  • Conduct thorough assessments of participants' nutritional status at enrollment, annually, and as needed.
  • Share relevant participant information with the Interdisciplinary Team to ensure comprehensive care.
  • Assist in planning dietary supplements for high-risk participants.
  • Report observations and participant changes, including behavioral changes, to ADH staff clinicians.
  • Participate in family meetings and case conferences as needed.
  • Provide follow-up treatment and teaching as necessary.
  • Make home visits as requested.
  • Interact with participants, caregivers, and families in a professional and courteous manner.
  • Interact with other providers and community professionals in a courteous and professional manner.
  • Maintain accurate records and documentation in a timely manner.
  • Ability to pass a fit test, as the position requires mask use where a tight seal is critical.
Additional Responsibilities:
  • Participate in Quality Improvement projects and assessments as needed.
  • Participate in Element Care Committees as required and communicate relevant information as necessary.
  • Complete required annual Health Stream trainings in a timely manner.
  • Adhere to all policies and procedures and support the Primary Vision Statement.
  • Adhere to the confidentiality policy of Element Care, including HIPAA guidelines for participant information.
  • Be flexible to perform other related tasks as reasonably requested.
Essential Job Requirements:
  • Master of Science Degree in Nutrition (preferred)
  • Registered Dietitian through CDR and current state licensure in the Commonwealth of Massachusetts.
Experience:
  • Two years of experience working in a community setting.
  • One year of experience working with the geriatric population.
Required Skills:
  • Ability to convey information clearly and succinctly to all employees.
  • Ability to lead independently and work well with others.
  • Ability to work within a team concept and be considerate of co-workers and colleagues.
Preferred Skills:
  • Ability to pay attention to detail and be a self-starter.
  • Ability to quickly recognize situations and problems before they escalate.
