Home Health Aide

CenterWell Home Health is seeking a compassionate and skilled Home Health Aide to join our team. As a Home Health Aide, you will provide personalized care and support to patients in their homes, helping them to maintain their independence and quality of life.
Responsibilities- Provide direct patient care under the supervision of a registered nurse, including assistance with daily living activities, personal care, and mobility.
- Assist patients with self-administered medications, including opening bottle caps, reading medication labels, and observing patients take their medication.
- Take accurate temperature, pulse, and blood pressure measurements, and recognize and report abnormal results to the supervisor.
- Help patients maintain good personal hygiene, including bathing, grooming, skin care, shaving, oral care, nail/foot care, and other activities.
- Assist with feeding patients, including communicating basic principles of nutrition, observing and recording food and fluid intake, and safely positioning patients for meals.
- Assist with patient toileting, including use of bed pan/urinal, changing and positioning catheter bags, and bag change procedures on well-regulated ostomies.
- Provide necessary skills to safely assist patients with mobility, exercises, positioning/turning, transfers, and ambulation, as per the Plan of Care and CenterWell Home Health policy.
- Document interactions with patients, caregivers, doctors, and other staff members, including legible, thorough, and timely documentation.
- Practice acceptable infection control principles, providing a clean, safe, and comfortable environment for patients.
- High school diploma or equivalent.
- Completion of Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
- Must meet applicable state certification requirements.
- A valid driver's license, auto insurance, and reliable transportation are required.
- Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency.
- At least one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency.
CenterWell Home Health offers a competitive compensation package, including a salary range of $32,500 - $44,700 per year, paid time off, 401(k) retirement savings plan, employee assistance program, business travel and accident insurance.
