Job Title: Self-Direction Direct Support Professional

At Central New York Quest Incorporated, we are seeking a dedicated and compassionate Direct Support Professional to join our team. As a Direct Support Professional, you will play a vital role in supporting individuals with disabilities to live independently and achieve their goals.

Key Responsibilities:
  • Establish Respectful Relationships: Develop meaningful relationships with individuals using a person-centered approach, getting to know them through assessment, observation, and discovery.
  • Assess and Support Daily Living Skills: Evaluate an individual's skills and capacity for skill development in their home and community, providing teaching and assistance as needed.
  • Promote Advocacy and Choice: Support individuals in understanding their rights and responsibilities, advocating for their needs, and making informed choices.
  • Communicate Effectively: Assist individuals in communicating with their support team, including families, natural supports, and community resources.
  • Respond to Crisis Situations: Use good judgment to prevent, intervene, resolve, and report crisis situations to ensure the safety and well-being of individuals.
  • Maintain Records and Documentation: Keep accurate records and documentation for each consumer served, submitting monthly notes by the 5th of each month for review.
  • Adhere to Agency Policies: Report all incidents to Central New York Quest's Director of Programs and follow incident reporting procedures as outlined in Central New York Quest's policies.
  • Maintain Confidentiality: Protect the confidentiality of all work-related information.
  • Meet Training Requirements: Complete OPWDD and agency training requirements, submitting training certificates to the Fiscal Intermediary.
  • Act in Accordance with Agency Mission: Demonstrate a commitment to Central New York Quest's Mission and values.
