Job Summary

We are seeking an experienced and team-oriented RN Case Manager to join our Central Facilities Team at Empath Health Hospice.

The RN Case Manager will create and maintain a plan of care, serving as the coordinator for patient and family interaction with Tidewell for the duration of the patient's hospice experience.

Key Responsibilities
  • Develop and implement individualized care plans that meet the unique needs of each patient
  • Coordinate patient and family interactions with Tidewell, ensuring seamless care delivery
  • Provide education and support to patients, family members, and caregivers on disease management, palliative interventions, and end-of-life care
  • Collaborate with interdisciplinary teams to ensure comprehensive care
  • Review and update care plans, physician orders, and other records as needed
Requirements

To be successful in this role, you will need:

  • A graduate degree in nursing from an accredited program
  • A current Florida RN license
  • At least one year of experience in an acute-care hospital environment providing direct patient care
  • Previous experience as a hospice RN is preferred
