Empath Health Hospice is seeking an experienced RN Case Manager to join our Central Facilities Team in Sarasota County. The successful candidate will create and maintain a plan of care, coordinating patient and family interaction with Tidewell for the duration of the patient's hospice experience.

Key Responsibilities:

  • Develop and implement a plan of care for patients and their families
  • Coordinate with healthcare providers to ensure seamless care delivery
  • Provide education and support to patients and their families
  • Review and update care plans, physician orders, and other records

Requirements:

  • Graduate of an approved nursing program
  • Current Florida RN license
  • Minimum one year experience in an acute-care hospital environment
  • Previous experience as a hospice RN preferred
