Case Manager RN

CVS Health is seeking a skilled Case Manager RN to join our team. As a Case Manager RN, you will be responsible for coordinating care for members with complex medical needs, ensuring they receive the best possible outcomes.
Key Responsibilities- Act as a liaison between members, employers, providers, and insurance companies to ensure seamless care coordination.
- Develop and implement individualized care plans to optimize member outcomes and facilitate timely return to work or optimal functioning.
- Assess and analyze member medical and vocational status to identify areas for improvement and develop strategies for enhanced wellness and medical outcomes.
- Communicate effectively with members, providers, and other stakeholders to ensure timely and effective care coordination.
- Prepare and maintain accurate documentation of case work activities and outcomes.
- Collaborate with internal multidisciplinary teams to ensure comprehensive care and support for members.
- Minimum 3+ years of clinical experience, preferably in diabetes, CHF, CKD, post-acute care, hospice, palliative care, or cardiac care.
- Minimum 2+ years of case management, discharge planning, and/or home health care coordination experience.
- Ability to travel occasionally within Sarasota, FL.
- Active state license in good standing within the region where job duties are performed.
- Certified Case Manager (CCM) certification.
- Excellent analytical and problem-solving skills.
- Effective communication, organizational, and interpersonal skills.
- Ability to work independently and efficiently in a fast-paced environment.
