The Division of Hospital Medicine (DHM) at Mount Sinai is looking for a full-time salaried Physician to attend to Post-Acute Care patients at a skilled nursing facility. This position is an excellent opportunity to join the Mount Sinai Department of Medicine, specifically within the Hospitalist Division, with primary responsibilities at the skilled nursing facility. The facility is recognized as one of the largest and most progressive providers of both facility and community-based Long Term Care services in the region, offering both post-acute rehabilitation and long-term care.
In this role, you will have the opportunity to collaborate with fellow hospitalists across the Mount Sinai network on scholarly initiatives, engage with Geriatric fellows and medical students, and contribute to medical education, performance improvement, and research efforts. Enjoy a favorable work-life balance with a daytime schedule from Monday to Friday.
Qualifications:
- Medical degree with training in Internal Medicine or Family Medicine.
- New York State Medical License.
- Board Eligible or Board Certified in Internal Medicine or Family Medicine.
- Preferred additional certification in Geriatrics and/or Palliative Medicine.
- Strong commitment to the Mount Sinai Health System and the communities served.
- Excellent communication skills and a strong bedside manner.
- Desire to work in a team-oriented, performance-driven environment.
- Conduct comprehensive evaluations of all new patients.
- Proactively manage chronic medical conditions to enhance recovery.
- Identify and address new medical issues that may hinder recovery.
- Collaborate with the interdisciplinary team for weekly patient meetings, documenting progress and planning.
- Participate in patient and family meetings to facilitate effective communication and discharge planning.
- Ensure patient goals are integrated into rehabilitation and discharge plans.
- Work effectively with patients, families, and teams to achieve established goals.
- Recognize when clinical progress is not meeting goals and intervene appropriately.
- Facilitate seamless care transitions through direct communication with other healthcare providers.
- Engage in performance improvement activities related to Post-Acute care.
- Fulfill scheduled on-call responsibilities.
- Complete and document advance care planning using relevant documentation.
- Adhere to principles of Geriatric and Palliative care in all medical practices.
- Comply with all applicable laws, regulations, and facility policies.
- Mentor and precept medical trainees, including Geriatrics fellows and medical students.
- Work towards defined clinical metrics set by the medical department.
The compensation range for this position is between 180K and 280K, exclusive of bonuses or benefits.
