Job Responsibilities
  • Provide exceptional patient care, demonstrating empathy and compassion in all interactions
  • Develop and implement individualized care plans, collaborating with interdisciplinary teams to ensure optimal patient outcomes
  • Assess patient needs, identifying opportunities for education and support to promote healthy behaviors and self-management
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless transitions and coordination of care
  • Participate in quality improvement initiatives, contributing to the development of best practices and evidence-based guidelines
Required Qualifications
  • Graduated from an accredited nursing program
  • Registered Nurse in the state of Illinois
  • CPR - Basic Life Support certification within 30 days of employment
  • Current certification appropriate to work area
