Job Summary
The Psychiatric Care Coordinator plays a vital role in ensuring the smooth transition of patients from our care to their next stage of treatment. This position is responsible for coordinating discharge planning and social services, working closely with multidisciplinary teams to identify patients' needs and develop comprehensive plans for their ongoing care. The ideal candidate will have a strong background in social work or a related field, with a minimum of three years of experience providing direct service to patients.

Key Responsibilities
- Coordinates discharge planning and social services for patients, identifying those who require psychiatric/mental health services after discharge and developing tailored plans to meet their needs.
- Works collaboratively with multidisciplinary teams to ensure seamless transitions between care settings and facilitate communication among healthcare providers.
- Conducts intakes, pre-admission assessments, and admission forms, as well as monitors clinical information to support psychosocial assessments, treatments, education, and continuity among healthcare providers.
- Enhances patient safety by maintaining a clean and safe environment, following regulatory guidelines, and adhering to infection control policies.
- Collaborates with patients and families to develop personalized plans of continuing care compatible with patient needs, goals, and financial resources.

Qualifications
- Minimum Bachelor's degree in social work or a related field.
- Three years of experience providing direct service to patients, knowledge of the mental health code, and understanding of normal characteristics of growth and development throughout the life span.
- Completion of Management of Aggressive Behavior Certification or CPI (yearly as offered).
