Primary Care Physician

We are seeking a highly skilled Primary Care Physician to join our team at CenterWell Senior Primary Care. As a member of our care team, you will have the opportunity to provide proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, minor injury treatment and more.
Our unique care model focuses on personalized experiences, taking time to listen, learn and address the factors that impact patient well-being. Our integrated care teams, which include physicians, nurses, behavioral health specialists and more, spend up to 50 percent more time with patients, providing compassionate, personalized care that brings better health outcomes.
As a Primary Care Physician, you will be responsible for evaluating and treating center patients in accordance with standards of care. You will follow level of medical care and quality for patients and monitor care using available data and chart reviews. You will also assist in the coordination of patient services, including but not limited to specialty referrals, hospital and SNF coordination, durable medical equipment and home health care.
You will act as an active participant and lead with the care team through daily huddles. You will help Regional Medical Director (RMD), Physician and Center Administrator in setting a tone of cooperation in practice by displaying a professional and approachable demeanor.
You will complete all medical record documentation in a timely manner working with a quality-based coder to optimize coding specificity. You will follow policy and protocol defined by Clinical Leadership. You will meet with RMD about quality of care, review of outcome data, policy, procedure, and records issues.
You will participate in potential growth opportunities for new or existing services within the Center. You will participate in the local primary care 'on-call' program of CenterWell Senior Primary Care. You will assure personal compliance with licensing, certification, and accrediting bodies.
This role is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance. The compensation range for this position is $115,200 - $158,400 per year, with opportunities for bonuses.
