About the Job

This role will work closely with the HBMC interdisciplinary team to deliver high-end medical service and compassionate care to complex, chronic patients.

The successful candidate will be responsible for developing and carrying out patients' goals of care in all phases of the patient journey, collaborating with PCPs, HBMC interdisciplinary teammates, and consultants as needed.

Key Responsibilities
  • Conduct initial comprehensive assessments on newly enrolled patients and provide ongoing care thereafter
  • Develop patient management care plans upon enrollment and update them as needed when changes in condition warrant or following hospitalization
  • Meet with patients and/or their legal representatives to review newly developed or modified care plans; involve PCPs or supervising physicians in these meetings when applicable
  • Prioritize patient care and make decisions to ensure timely interventions and prevent unnecessary acute admissions
Requirements
  • Accredited program completion in Nursing or Physician Assistant
  • Current state RN/NP or PA license
  • Certification through reputable organizations such as AANP, ANCC, or NCCPA
  • Reliable transportation access; compliance with Optum's Motor Vehicle Safety policy required
Preferred Qualifications
  • At least 2 years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care/emergency/urgent care
  • Familiarity with long-term care, community resources, and cost-effective alternatives for patient care delivery systems
  • Proficiency in CPT, ICD-10, and HCPC codes
Salary and Benefits

The estimated annual salary for this role is $88,000 - $173,200.
