Job Description

This position involves providing comprehensive assessments and developing patient management care plans. The ideal candidate will have experience working with geriatric patients and be comfortable with basic procedures such as IV placement and wound care.

Responsibilities:

  • Perform initial comprehensive assessments on newly enrolled patients and provide ongoing care thereafter
  • Develop and update patient management care plans as needed
  • Maintain effective communication with patients and healthcare professionals to ensure seamless care delivery
  • Collaborate with interdisciplinary teams to implement high-quality care standards

Requirements

  • Completion of Nurse Practitioner or Physician Assistant accredited program
  • Current state RN/NP or PA license
  • Certification through American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA)
