Job Summary

We are seeking a highly motivated and compassionate Community Health Worker to join our team at Samaritan Healthcare. As a Community Health Worker, you will play a vital role in promoting healthy lifestyles and providing education and support to patients in our community.

Key Responsibilities
  • Provide outreach and education to patients on healthy lifestyle choices and disease management.
  • Identify patient needs and provide relevant resources and support to promote self-management and quality outcomes.
  • Collaborate with care coordinators and healthcare professionals to ensure seamless care transitions and coordination.
  • Conduct home visits and gather information on home safety and social determinants of health.
  • Stay up-to-date on industry trends and best practices in healthcare and community outreach.
Requirements
  • High School Diploma or equivalent.
  • Active license in the State of Washington as a Medical Assistant-Certified, preferred.
  • Valid Washington State Driver's License and reliable transportation.
  • Current (Healthcare Provider [HCP] level) CPR certification.
  • Experience or education in patient coaching, education, and training, preferred.

  • Critical thinking skills: Seeks resources for direction, when necessary.
  • Performs actions that demonstrate accountability.
  • Demonstrates competency in ability to care for customers/patients across the age continuum.
  • Demonstrates a basic understanding of the essential function representative of Seven Domains of Care Coordination in the primary care setting.
Physical Requirements
  • Frequent prolonged standing/walking.
  • Occasional lifting, reaching, kneeling, bending, stooping, pushing, and pulling.
  • Occasional heavy lifting (lift/carry up to 50 lbs.).
  • Manual dexterity and mobility.
