Position Overview:

Eventus WholeHealth is dedicated to providing a comprehensive care model that addresses the needs of medically vulnerable adults, ensuring exceptional outcomes for those we serve. Our approach is grounded in a framework of excellence, integrity, person-centered care, stewardship, and collaboration. We deliver services to adults residing in skilled nursing and assisted living facilities, as well as private homes, through a network of healthcare professionals including Physicians, Nurse Practitioners, Physician Assistants, Psychologists, and Licensed Clinical Social Workers, supported by Clinical and Administrative Staff.

Role Responsibilities:
  • Provider Support:
    • Facilitate the clinical provider's focus on patient care throughout the day.
    • Ensure each patient is evaluated according to clinical protocols.
    • Act as a liaison between providers and families/staff to enhance patient management.
    • Document encounters and time for Care Coordination Management (CCM) and Behavioral Health Integration (BHI) services.
    • Maintain and update scheduling rosters daily for accuracy.
    • Assist with any immediate changes to scheduled visits due to urgent needs.
    • Prepare clinical documentation for assigned providers on a daily basis.
    • Conduct medication reconciliation for all scheduled patients.
    • Input relevant facility chart data into patient encounter notes.
    • Enter lab and diagnostic results into patient encounters.
    • Organize and sort clinical paperwork for clinician review.
    • Facilitate telehealth sessions as assigned.
  • Facility Support:
    • Share patient schedules with essential facility staff as necessary.
    • Assist with triage needs as directed by the assigned provider.
    • Ensure all required paperwork is routed appropriately.
    • Participate in meetings to relay information between facility staff and Eventus providers.
    • Support discharge planning for clinical services as needed.
  • Patient Support:
    • Maintain confidentiality of patient information at all times.
    • Document encounters and time for CCM and BHI services.
    • Regularly check communications to address patient needs.
    • Inform patients and families of recent visits and new orders.
    • Communicate transitions of care to enhance patient education and support.
Additional Expectations:
  • Adhere to Eventus WholeHealth standards, policies, and procedures.
  • Maintain a professional appearance in accordance with the dress code.
  • Report to the Lead Medical Assistant for additional assignments when tasks are completed.
  • Seek further training when uncertain about any assignment.
Qualifications:

- High School Diploma or equivalent
- Valid driver's license and reliable transportation
- Willingness to travel up to 1 hour from home
- Certified Medical Assistant credential preferred
- Typing speed of 45 words per minute or greater
- Experience in medical scribing is advantageous
