About the Role

We are seeking a skilled and compassionate Hospice Nurse to join our team at Carilion Clinic. As a key member of our Hospice team, you will play a vital role in providing high-quality patient care and support to individuals and their families.

Key Responsibilities
  • Conduct comprehensive assessments of patients and develop individualized plans of care
  • Coordinate symptom management and provide follow-up visits as needed
  • Collaborate with the Interdisciplinary Team (IDT) to ensure seamless care coordination
  • Participate in IDT meetings and contribute to the development of patient care plans
  • Provide education and support to patients and their families
  • Document patient information accurately and timely in electronic medical records
  • Participate in performance improvement activities and contribute to the development of quality improvement initiatives
Requirements
  • Graduate of an accredited Registered Nurse Program
  • One year of experience in hospice, home health, oncology, or medical surgical nursing
  • Currently licensed as a Registered Nurse in the state of Virginia or approved compact state
  • Valid driver's license, automobile insurance, and personal vehicle for making home visits
  • AHA BLS-HCP certification
  • Clear criminal records check
