Avamere Communities is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Avamere Rehabilitation of Cascade Park in Vancouver, WA.
About the Role:We are looking for a dedicated and caring LPN to provide high-quality patient care in our skilled nursing community. As an LPN, you will be responsible for coordinating with nursing staff and support personnel to plan and implement resident care plans, administering medications, and maintaining accurate records.
Responsibilities:- Complete required record keeping and coordinate with nursing staff to plan resident care plans
- Administer medications and review medication cards for accuracy
- Provide leadership to nursing personnel and make daily rounds to ensure direct care is provided in accordance with resident care plans
- Participate in facility surveys and maintain confidentiality of resident care information
- Nursing degree from an accredited college or university
- Current, unencumbered LPN license in the state of Washington
- Knowledge of nursing and medical practices, as well as laws and regulations pertaining to nursing care facilities
- Experience with Electronic Medical Records and computer documentation systems
