Job Summary:
The Case Manager I role at Tarzana Treatment Centers, Inc. is a dynamic opportunity to work with patients seeking treatment for substance use disorders and other behavioral health issues. As a member of our multi-disciplinary team, you will provide case management services, conduct outreach and follow-up activities, facilitate discharge-planning efforts, and collaborate with other team members to develop comprehensive treatment plans.

Salary and Benefits:$25.75 - $29.50 per hour DOE (dependent on experience). About the Role:This Case Manager I position requires strong communication and interpersonal skills, with the ability to build rapport with patients and their families. Responsibilities:
  1. Provide case management services to patients assigned to individual caseload.
  2. Collaborate with other multi-disciplinary team members to develop comprehensive Individualized Treatment Plans.
  3. Facilitate discharge-planning efforts, commencing at time of admission on all assigned patients.
  4. Makes direct contact within agency and with outside entities as appropriate, to assist patients in resolution of legal, financial, housing, medical, psychiatric, and discharge planning issues.

Qualifications:
  • For AOD Programs: 1 year experience in a Behavioral Healthcare setting and registration or certification with a DHCS approved state level counselor certification agencies.
  • For all other Programs: 1 year experience in a Behavioral Healthcare setting (HIV programs) and DVSS Certification for DV specific staff.
  • Bilingual English/Spanish preferred.
  • Valid California driver's license and insurance.
