Hospice Care Assistant

Are you passionate about delivering exceptional care to patients and their families? Do you have a knack for empathy and effective communication? We're seeking a compassionate and skilled Hospice Licensed Nursing Assistant (LNA) to join our dynamic team at Cornerstone VNA.
Job SummaryAs a Hospice Licensed Nursing Assistant (LNA), you will provide basic client/patient care, assist with activities of daily living, and work collaboratively with our interdisciplinary team to deliver high-quality care. If you're a caring and dedicated individual with a passion for making a difference, we encourage you to apply.
Responsibilities- Provide basic client/patient care, including assistance with eating and nutrient intake
- Aid clients/patients with activities of daily living and instrumental activities of daily living in accordance with the individual plan of care
- Carry out treatments on patients' plan of care, such as home exercise programs, stable wound care, and range of motion
- Take, read, and record vital signs and collect samples and specimens for tests
- Work collaboratively with members of the healthcare team to provide excellent service for patients
- LNA license and registration completed and filed with New Hampshire; Maine (CNA) licensure is also preferred and required if seeing patients/clients in Maine
