About the Role

Moments Hospice is seeking a compassionate and skilled Registered Nurse to join our team in a On-Call capacity. This position involves working 21 days a month, with 2 weekends and 5pm-8:30am shifts on weekdays. Our Triage Team ensures that on-call duties are limited to times when a RN is needed to visit patients/families.

Key Responsibilities
  • Provide skilled professional nursing care to patients, families, and caregivers
  • Oversee and provide primary care, including clinical care, palliative care, and symptom control, and identify other physical, psychological, social, and spiritual needs for assigned patients
  • Regularly assess patients for nursing needs and implement the Plan of Care Provisions to meet their needs
  • Work closely with the interdisciplinary team to ensure all patient and family care needs are being met
  • Collaborate with the Patient Care Manager and Interdisciplinary Team in planning, implementing, and evaluating medical and nursing care
Qualifications
  • Current and unrestricted Registered Nurse licensure
  • Bachelor's degree and C.H.P.N. (Certified Hospice and Palliative Nurse) certification preferred
  • Minimum of 1 year general nursing experience, preferably with current Hospice, home health, medical, surgical, or critical care experience
  • Experience with pain and symptom management interventions
  • Ability to work within an interdisciplinary setting
  • Understanding of Hospice philosophy and principles of death/dying
