About the Role

We are seeking a skilled Registered Nurse to join our Home Care Coordination and Intake team as a Hospice Evaluator RN. In this role, you will be responsible for facilitating patient discharge from the facility setting to hospice care services in the appropriate setting and level of care.

Key Responsibilities
  • Evaluate patient eligibility, needs, and care resources for hospice services
  • Coordinate all services, including professional, paraprofessional services, durable medical supplies, equipment, etc.
  • Collaborate with facility care coordinators, social workers, and physicians to find solutions for challenging patients requiring hospice care
  • Complete comprehensive clinical assessments and develop hospice plans of care with patients and caregivers
Requirements
  • Valid NYS License as an RN required
  • Diploma or Associate's Degree in Nursing required; Bachelor's Degree in Nursing preferred
  • Experience in working in partnership with the interdisciplinary healthcare team
  • Experience utilizing an Electronic Medical Record and Microsoft office
