Job Summary

We are seeking a highly skilled LPN to join our team as a Medication Care Manager. As a key member of our care team, you will be responsible for providing high-quality care and services to our residents by administering medication and treatments in a safe and organized manner.

Responsibilities

* Administer medications and treatments to residents in accordance with their individualized care plans
* Conduct Service and Health Updates as directed by the Resident Care Director
* Participate in the development of Individualized Service Plans (ISPs) and monthly updates
* Maintain accurate and up-to-date medication records
* Collaborate with the care team to ensure resident safety and well-being

Requirements

* Current LPN licensure in the state of practice
* Previous experience working with seniors is preferred
* Strong communication and interpersonal skills
* Ability to work effectively in a team environment
* Commitment to providing high-quality care and services to residents
