Private Duty Nurse

We are seeking a skilled Registered Nurse to join our team at Almost Family, a part of LHC Group. As a Private Duty RN, you will provide high-quality clinical services to patients in their homes, working closely with other healthcare professionals to achieve and maintain continuity of patient care.
Key Responsibilities- Provide clinical nursing assessments and develop individualized plans of care in accordance with federal and state program requirements.
- Ensure patient eligibility and medical necessity for services, as defined by payer source and agency policy.
- Collaborate with other community providers to develop and revise plans of care and service plans.
- Initiate plan of care revisions as needed, with physician approval.
- Current RN licensure in the state of practice.
- One year of clinical experience as an RN, or recent clinical experience as an LPN (in some states).
- Current CPR certification.
