Hospice and Palliative Care Nurse Practitioner

We are seeking a skilled Hospice and Palliative Care Nurse Practitioner to join our team at Andwell Health Partners. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality, patient-centered care to individuals with serious illnesses.
Responsibilities- Assist with family meetings to clarify goals of care, code status, and transition to hospice as needed.
- Coordinate care among internal and external clinicians and healthcare providers to ensure seamless transitions and optimal patient outcomes.
- Provide on-call services via phone for hospice patients, both in the community and at the hospice house.
- Participate in care coordination meetings, including ICU rounds, St. Mary's oncology, hospice house, and palliative care interdisciplinary team meetings.
- Act as a resource to staff, patients, family members, and attending providers regarding pain and symptom management.
- Serve as attending or covering provider for patients at the Hospice house, responsible for ordering medications, treatments, and testing as appropriate.
- Perform palliative care initial consultations and follow-up visits in the community or acute care setting as requested by the primary care team.
- Master's degree in nursing and certification by the American Academy of Nurse Practitioners (AANP).
- One year of recent clinical nursing experience required.
- Current R.N. license in the State of Maine.
- Current CPR Card.
- Must have a reliable and insured automobile and a current Maine driver's license.
- Independent malpractice insurance strongly advised.
