Job Overview

Andwell Health Partners is seeking a skilled Nurse Practitioner to join our team in providing high-quality, patient-centered care to individuals in need of hospice and palliative care services.

Key Responsibilities:

  • Collaborate with interdisciplinary teams to develop and implement comprehensive care plans.
  • Provide on-call services to patients and families, ensuring timely and effective support.
  • Participate in care coordination meetings to ensure seamless transitions and optimal patient outcomes.
  • Act as a resource for staff, patients, and families on pain and symptom management strategies.
  • Order medications, treatments, and testing as necessary for patients at the Hospice house.
  • Perform palliative care initial consultations and follow-up visits in the community or acute care setting.

Requirements:

  • Master's degree in nursing and certification by the American Academy of Nurse Practitioners (AANP).
  • One year of recent clinical nursing experience required.
  • Current R.N. license in the State of Maine.
  • Current CPR Card.
  • Reliable and insured automobile and a current Maine driver's license.
  • Independent malpractice insurance strongly advised.
