Hospice Admission Nurse

Hospice Admission Nurse Role
The Hospice Admission Nurse will be an expert in the admission process and well-versed in hospice eligibility. They will assist with hospice talks, providing information about the hospice benefit for potential hospice patients and their families.
Key Responsibilities:
- Provide skilled nursing services to patients in their own homes under the direction of a physician's plan of care.
- Develop plans of care with realistic and measurable outcomes in collaboration with the patient and family.
- Assess and identify the patient's holistic healthcare needs.
- Provide education and promote patient rights and responsibilities for informed decision-making in the plan of care.
Requirements:
- One year of recent clinical nursing experience required.
- ANA certification in Home Health or Community Health Nursing encouraged.
- A reliable car with insurance.
