About the Job

We are recruiting for a Unit-Based Clinical Pharmacist in Cardiology to work in the evening shift at the Hospital of the University of Pennsylvania (HUP). As a member of our team, you will be responsible for preparing and dispensing medications, providing drug information, and ensuring continuity of pharmaceutical care.

Key Responsibilities
  1. Preparation and Dispensation of Medications: Prepare and dispense medications using appropriate techniques to ensure outcomes of drug therapies and following health system policies and procedures.
  2. Drug Information: Provide concise, applicable, and timely responses to drug information requests from healthcare providers and patients.
  3. Continuity of Pharmaceutical Care: Ensure continuity of pharmaceutical care to and from acute and ambulatory patient-care settings.
  4. Adverse Drug Events: Participate in the established process for assessing, managing, and reporting Adverse Drug Events (ADEs).
Requirements
  • Bachelor of Science in Pharmacy or Doctor of Pharmacy from an accredited College of Pharmacy.
  • Current pharmacist license in the state of Pennsylvania or eligible for licensure.
  • PGY-1 Pharmacy Practice Residency or 2 years of practice experience (preferred).
