About the Role

We are seeking a skilled Hospital Social Worker to join our team at MultiCare Health System. As a key member of our care coordination team, you will play a vital role in ensuring seamless transitions of care for our patients.

Responsibilities
  • Provide cross-continuum care coordination, including clinic, provider practice, urgent care, phone care agency, skilled care, and other sites of care as appropriate.
  • Offer customer service to patients, families, physicians, APPs, and care team members, including discharge milestones and progression to next level of care.
  • Model and orchestrate consistent, professional communications and maintain consistent and proactive communication patterns with colleagues and staff.
  • Delegate responsibilities appropriately while remaining accountable for outcomes.
  • Ensure patients and families experience medically appropriate transitions of care.
Requirements
  • Master's degree in Social Work with a healthcare-based practicum.
  • Licensed in the state of Washington as a Social Worker Associate Independent Clinical (LSWAIC) with a commitment to obtain LICSW within four years.
  • Hospital or equivalent experience preferred; experience prior to obtaining a Master's degree may be considered.
  • Health industry knowledge preferred.
  • Healthcare financial assistance knowledge preferred.
