Job Summary

We are seeking a highly motivated and compassionate Community Health Worker to join our team at Ochsner Health. As a key member of our healthcare team, you will play a vital role in providing patient-centered care and support to individuals and communities in need.

Key Responsibilities
  • Assist with chronic care management by providing patient follow-up services, documenting patient outcomes, and educating patients on healthy lifestyle choices.
  • Act as a mentor and coach to less experienced Community Health Workers, assisting in their training and certification.
  • Work with patients to assess and address their needs, providing culturally aware social support and assistance with capacity building and motivational support.
  • Engage in intra- and inter-agency collaboration to improve individual and community health, coordinating patient care across health systems and navigating community resources.
  • Provide community health education and outreach to inform individuals, families, and local agencies about health and wellness services, and assist with health fair events.
Requirements
  • High School diploma or equivalent required; 1 year of experience working or volunteering in community-based programs with direct client interaction preferred.
  • Community Health Worker training certificate must be obtained within 1 year of hire.
  • Strong interpersonal skills, ability to work with community members from diverse backgrounds, and proficiency with Windows-style applications and software packages specific to the role.
