Community Health Advocate

We are seeking a highly motivated and compassionate Community Health Worker to join our team at Ochsner Health. As a Community Health Worker, you will play a vital role in promoting health and wellness in our community by providing patient follow-up services, educating patients, and facilitating support groups.
The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively with patients from diverse backgrounds. You will be responsible for assisting with chronic care management, working with patients to assess and address their needs, and engaging in intra- and inter-agency collaboration to improve individual and community health.
Key Responsibilities:
- Assist with chronic care management by providing patient follow-up services and documenting patient outcomes
- Work with patients to assess and address their needs, both in person and telephonically
- Engage in intra- and inter-agency collaboration to improve individual and community health
- Provide community health education and outreach to inform individuals, families, and local agencies about health and wellness services
- Assist with health fair events and provide culturally aware social support and assistance with capacity building/motivational support
Requirements:
- High School diploma or equivalent required
- 1 year of experience working or volunteering in community-based programs with direct client interaction preferred
- Community Health Worker training certificate must be obtained within 1 year of hire

Light Work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
Normal routine involves no exposure to blood, body fluid or tissue and as part of their employment, incumbents are not called upon to perform or assist in emergency care or first aid.
The incumbent has no occupational risk for exposure to communicable diseases.
Because the incumbent works within a healthcare setting, there may be occupational risk for exposure to hazardous medications or hazardous waste within the environment through receipt, transport, storage, preparation, dispensing, administration, cleaning and/or disposal of contaminated waste.
The risk level of exposure may increase depending on the essential job duties of the role.
