Job Summary:

We are seeking a highly skilled and dedicated individual to fill the role of ACT Program Coordinator/Team Assistant at RHA Health Services. This position is responsible for developing and maintaining strong positive relationships with key stakeholder groups, ensuring all stakeholder communications are proactive and responsive, and assisting in the overall management and functioning of the program.

Key Responsibilities:

  • Develops and maintains positive relationships with key stakeholders, ensuring stakeholder satisfaction with the company by maintaining proactive, responsive communication.
  • Maintains regular, proactive communication with key stakeholder groups, and builds new relationships as needed to further the company's mission, vision, and referral sources.
  • Directs communication with management and colleagues, establishing and maintaining frequent and regular communication with program director and staff to assure smooth program functioning.
  • Participates in all regulated trainings or trainings as they apply to their position as needed, and participates in group supervision and staff meetings in accordance with program and company requirements.
  • Accepts new referrals from outside sources and confirms eligibility for services, assists program in preparing and submitting required paperwork to coordinate referrals/authorization for services to the Department of Public Welfare, managed care companies, and/or other governmental bodies in a timely manner.
  • Runs EVS checks upon intake, and the 1st and 15th of every month to monitor client's insurance eligibility, complete service authorizations, track regulated paperwork, and represent the company as a liaison to county offices, managed care companies, Department of Public Welfare, schools, and all other providers involved with client care.
  • Monitors services provided to ensure services are within the prescribed authorization period and within the approved limit of hours, communicates with parent/guardian regarding client progress/concerns through quality-of-service verification calls, and monitors effectiveness of treatment protocols, through weekly Electronic health records checks, monitored on a shared spreadsheet, to assure compliance with regulated timeframes for all intakes and ongoing assessments/annual forms and treatment plan due dates and report treatment concerns to supervisor.
  • Completes on-boarding and off-boarding of clients within electronic health record, schedules and manages psychiatrist schedule, sends re-engagement letters as applicable, runs various Electronic health records reports as it pertains program need, and performs responsibilities associated with the Mental Health Worker position, as directed.
  • Assists in the completion of bi-annual linkage letters, assists in preparation process prior to audits OMHSAS/TMACT, completes miscellaneous paperwork/reports as directed, maintains client confidentiality per company policy, HIPAA Privacy Rule regulations, and the Department of Public Welfare/CASSP principal guidelines, and adheres to company policies and procedures.
Requirements:

  • Complete criminal background, name checked in the registries (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities), drug testing, education verification and other credentialing based on position requirements, proof of employment history or references (if required), and positions that require driving, proof of driver's license, driver's insurance, and vehicle (if required for providing transportation for individuals).
