Job Overview

As a Program Coordinator with Greater Springfield Senior Services, Inc. (GSSSI), you will play a vital role in supporting the Community Transition Liaison Program (CTLP).

The estimated annual salary for this position is $40,000 - $50,000, depending on experience.

This full-time position requires 35 hours of work per week and is based in our Springfield office.

Key Responsibilities
  • Maintain accurate and up-to-date records of departmental and consumer data using manual and computerized methods.
  • Develop and maintain working knowledge of relevant State Home Care (SHC) and CTLP databases.
  • Support the Community Transition Liaison in assisting consumers during their transition from skilled nursing facilities back to their community.
  • Accompany the Liaison on nursing facility visits as needed.
  • Gather necessary documentation to assist consumers in applying for public benefits.
  • Assist consumers in completing and submitting housing applications.
  • Analyze and track statistical data as directed.
  • Work independently and collaboratively as part of a team.
  • Requires fluency in English and Spanish.

Requirements

  • High School Diploma.
  • 1 year of related work experience or interest in working with older adults or persons with disabilities transitioning from an institutional to a community setting.
  • Proficiency in computer skills including spreadsheets, word processing, and data entry (Windows, Word, Excel).
  • Able to operate a keyboard for at least 80-90% of the workday.
