Nurse Care Coordinator

Compensation: $76,000 - $78,000 plus annual bonus
This role offers a unique opportunity to work with DDN Consulting Services LLC as a Nurse Care Coordinator in the West Region. The successful candidate will be responsible for providing oversight, training, and documentation to ensure high-quality care for individuals with developmental and intellectual disabilities.
About the Role:
- Key Responsibilities:
- Develop plans to advocate for healthcare needs
- Write nursing reviews
- Monitor medical and dental records
- Review documentation to ensure certified staff follows procedures for reporting errors in medication administration
- Supervise the initial worksite for med administration by staff
- Conduct onsite training

Requirements:
- Active RN license in Connecticut
- Valid driver's license and reliable transportation
- 1-2 years of nursing experience preferred; experience with developmental disabilities is a plus
