Hospice Care Coordinator

About the PositionThis Hospice LPN role is a fantastic opportunity for a skilled nursing professional to join our team at Masonicare Corp in Avon, CT. As a Hospice Palliative Care LPN, you will provide high-quality care to patients and their families in the comfort of their own homes or extended care facilities.
You will work under the direction of the Nurse Case Manager and/or Clinical Manager, coordinating care with community and agency resources, and providing nursing care reflective of the agency's mission.
Key Responsibilities- Coordinate patient care with community and agency resources
- Provide skilled nursing care as per established plan of care and document clinical activities per agency policy
- Recognize and interpret symptoms and institute remedial measures within scope/limits of Licensed Practical Nurse in CT
- Administer medication and treatments as ordered by physician
- Reinforce appropriate interventions, medications, effects, and side effects to patient, caregiver, volunteers, and others as directed by RN Case Manager
- Graduate of NLN accredited school of nursing required
- Experience in home health care or hospice preferred
- LPN license and current drivers' license and auto insurance required
