Supplemental Health Care is seeking a skilled Physical Therapist to join our team in Saco, Maine. As a Physical Therapist with our company, you will have the opportunity to work with patients in their homes, providing personalized care and support to help them achieve their goals.
Job Description:- Provide physical therapy services to patients in their homes, including assessments, goal setting, and treatment planning.
- Develop and implement individualized treatment plans to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
- Communicate effectively with patients, families, and caregivers to ensure understanding and adherence to treatment plans.
- Document patient progress and outcomes in a timely and accurate manner.
- Graduate of an accredited Physical Therapy program.
- Licensed as a Physical Therapist in Maine.
- CPR certified or ability to obtain.
- Previous experience with computer charting.
- 6 months of previous home health care experience.
- Valid U.S. driver's license and reliable transportation.
