Position Overview:
This role is essential in providing reassurance and assistance during challenging times for clients.

As a Client Care Coordinator, you will collaborate with individuals to empower them in managing their own health care. Your responsibilities will include conducting clinical evaluations, resource assessments, and facilitating service coordination among healthcare providers and support networks.

In your capacity as a client advocate, you will engage with clients regularly and accompany them to healthcare appointments as necessary.

- Required qualifications include a Bachelor’s Degree in Social Work, Psychology, Human Services, Behavioral Sciences, or a related discipline
- Certification as a Medical Assistant with an Associate Degree
- Certification as a Community Health Representative (CHR) or Department of Health Certified Chemical Dependency Professional
- Minimum of 2 years of experience in nursing, healthcare, behavioral health, or human services (paid or volunteer)
