As a Family Support Coordinator at Communitas, Inc., you will play a vital role in empowering families who have sons and daughters eligible for DDS services. Your primary responsibility will be to provide home-based services to families, assisting them in identifying their needs and translating those needs into services and community supports.
Key Responsibilities- Inform and Assist Families: Inform families about available supports and services in their community and assist them in accessing these resources.
- Obtain Entitlement Benefits: Assist families in obtaining state and federal entitlement benefits, such as SSI, CommonHealth clinical/medical supports, and behavioral supports.
- Understand Special Education Laws: Have a thorough understanding of the Massachusetts Special Education Laws and their application to families.
- Collaborate with Agencies: Collaborate with other agencies, including the Department of Children and Families (DCF), to ensure comprehensive support for families.
- Supervise Support Staff: Supervise support staff working in families' homes to ensure high-quality services.
- Education: Bachelor's Degree in Social Work, Education, Human Services, or a related field is required. Master's Degree position is also available.
- Experience: Up to two years of family support case management experience preferred.
- Passion and Dedication: A passion and dedication to supporting people with intellectual and developmental disabilities is essential.
- Valid Driver's License: A valid driver's license, reliable transportation, and an acceptable driving record are required.
- Background Check: Must pass company background and reference checks.
