Service Coordinator

Apicha Community Health Center is seeking a highly motivated and organized Service Coordinator to join our team. As a Service Coordinator, you will play a critical role in ensuring the delivery of high-quality services to our clients.
Job Summary:We are seeking a Service Coordinator to provide triage services under the HIV Supportive Services Program. The successful candidate will be responsible for ensuring client eligibility, coordinating centralized intake, and making referrals as needed.
Key Responsibilities:- Adhere to AIDS Institute requirements for the provision of HIV Supportive Services program.
- Conduct intake assessments and screen clients' needs, working closely with the case manager and peer navigator to ensure implementation of service/care plans.
- Develop service plans that incorporate goals and objectives, frequency and duration of services, HIV-specific health and nutrition topics.
- Work closely with the Program Manager for Support Services to ensure appropriate services are delivered to program clients, including conducting HIV Support Services program orientation, screening/re-screening, and intakes.
- Act as a point person with HIV Support Services program staff to coordinate activities, including assisting the Program Manager for Support Services to ensure adherence to protocols and standards of Department of Support Services.
- Provide psychoeducation support and nutrition health education groups to program clients.
- Work closely with the case manager and peer navigator to coordinate the purchase and distribution of vouchers that meet program guidelines.
- Recruit and negotiate with vendors that will accept Apicha Food Voucher.
- Maintain financial and program variances of the program.
- Assist the Program Manager for Support Services to coordinate and attend all required training sessions and meetings.
- Work closely with Apicha CHC staff and other service providers to coordinate clients' care.
- Manage program client satisfaction and exit surveys, complete reports, and represent results.
- Manage and monitor program key deliverables to clients and meet program benchmarks.
- Maintain accurate records on services and activities, including attendance, registration, client interactions, and other required documentation according to protocols and standards of Apicha's Department of Support Services in designated database and reporting structures.
- Write and submit reports adhering to deadlines.
- Work as part of the team for quality assurance and continue quality improvement (CQI) tasks.
- Communicate effectively with clients, families, and the public to build trusting relationships across a broad range of socioeconomic and cultural backgrounds.
- Represent the agency at various venues to promote agency services.
- Other activities as assigned.
- BA/BS in a related field with two (2) years of qualifying experience required. An advanced degree in one of the qualifying fields may be substituted for one (1) year of qualifying experience.
- Experience working with target populations strongly preferred, including HIV/AIDS, individuals with Serious Mental Illness, serious substance use disorders (SUDs), and/or chronic hepatitis.
- Fluency in Spanish and/or Asian languages a plus.
- Excellent organization, written, verbal, and interpersonal communication skills in English required.
- Consistent team player capable of effectively operating within the organizational structure.
- Appreciation of and commitment to Apicha's mission and values required.
- Excellent customer service skills to assist in resolving member complaints.
- Must possess ability to interact with all levels of external and internal customers with a high level of professionalism and tact in representing Apicha CHC.
- Strong verbal and written communication skills – for both internal and external audiences.
- Computer literacy – ability to learn and adapt to different reporting platforms within required time frames.
- Ability to work effectively in stressful environments.
- Ability to work effectively in both collaborative and individual settings.
- Ability to work flexible hours, including some evenings and weekends.
