Inpatient Care Coordinator

As an Inpatient Care Coordinator - RN, you will play a vital role in coordinating patient care services and promoting effective resource utilization within our interdisciplinary team. Your responsibilities will include reviewing patient admission data, developing and implementing comprehensive care plans, assessing patients' goals and biophysical needs, and communicating utilization assessments to justify continued hospitalization. You will work closely with our nursing staff to ensure seamless care delivery, prioritize tasks, and maintain a patient-centered work environment. If you possess strong interpersonal skills, a solid understanding of nursing principles, and the ability to prioritize tasks effectively, we encourage you to apply for this rewarding role.
Qualifications:
• Accredited School of Nursing degree in Nursing
• Minimum 3 years of clinical nursing experience in an acute care hospital
• Basic knowledge of statistics
• Interpersonal skills to obtain and interpret information
• Skill and proficiency in applying technical principles, concepts, and techniques
• Ability to promote positive interpersonal relationships and patient-centered work environment
• Basic computer skills and effective prioritization skills
Working Conditions:
• Lifting and patient handling within specified limits
• Fine motor skills to grasp and control medical equipment
• Ability to move around area with frequent sitting and bending/stooping/reaching/kneeling
• Environmental conditions, including exposure to biohazard and airborne particles
• Must be able to complete tasks in a noisy environment
