The estimated salary for this role ranges from $46,900 to $70,360 per year, depending on experience and qualifications.

Job Description

We are looking for an experienced RN with excellent leadership skills to manage complex care services in our outpatient setting. The ideal candidate will have a minimum of 3 years of clinical nursing experience in an ambulatory or clinic setting and be proficient in applying highly technical principles, concepts, and techniques.

  • Develop and evaluate population management programs, tools, and delivery of care coordination services.
  • Risk-stratify patients according to protocols, assess patient data, and work within interdisciplinary care teams.
  • Create and implement care plans that meet identified needs and incorporate services to improve outcomes.
  • Delegate and oversee the care management of lower-risk patients and routine chronic disease population management tasks.

This role requires strong communication and interpersonal skills, with the ability to promote positive relationships with patients, families, and staff. A valid driver's license and ability to work in an independent and remote environment are also required.

Required Skills and Qualifications
  • Associate Degree Required: Nursing.
  • Bachelor's Degree Preferred: Bachelor of Science in Nursing or Master's Degree Preferred: Master of Science in Nursing.
  • Minimum of 3 years Required: Clinical nursing experience in an ambulatory or clinic setting.
  • Registered Nurse in state of practice.
  • Basic Life Support and Advanced Cardiac Life Support certifications.
