Nursing Care Coordination Opportunity

We are seeking a skilled Registered Nurse to join our part-time team at PeaceHealth. This 0.50 FTE position involves delivering high-quality nursing care and coordinating management of patients admitted to home care and clinic services. The successful candidate will have a graduate degree in nursing, at least two years of acute clinical or home health/hospice nursing experience, and current licensure in the state of practice.

Key responsibilities include:
  • Performing initial patient evaluations to develop individualized plans of care
  • Coordinating with the interdisciplinary team to ensure seamless care delivery
The ideal candidate will possess excellent communication and coordination skills, with a strong focus on patient-centered care. To be successful in this role, you will need to have BLS certification, a valid driver's license, and a vehicle in good working order.
