Responsibilities:

  • Provide high-quality, patient-centered care to individuals in their homes, promoting independence and dignity.
  • Conduct thorough assessments, develop and implement individualized care plans, and coordinate with healthcare teams to ensure seamless care transitions.
  • Collaborate with patients, families, and caregivers to educate and empower them to manage their health and well-being.
  • Participate in ongoing quality improvement initiatives to enhance patient outcomes and satisfaction.

Qualifications:

  • Graduate of an accredited school of practical nursing.
  • Licensed to practice in the state serviced.
  • Current CPR or BLS Certification.
  • Compliant with appropriate state certification/registration.
  • Proof of eligibility to pass medications.
  • One year hospital nursing experience.
